We recommend completing our new patient form prior to your first appointment if possible. Your cooperation in completing this questionnaire is essential for us to provide you with safe and appropriate dental care. We will send pre-authorization requests to your insurance provider and assist you with your claim form submissions to make the process as seamless as possible.
Rest assured, you will typically receive reimbursement for your dental claims within two to five business days after submission.

The full treatment cost must be paid at the time of your appointment. We accept payment by cash, debit or most major credit cards.
